Job Description

The Service Coordinator’s role is to assume responsibilty for the service management function within the assigned property, ensuring that residents are educated about and connected to available community resources. Will be responsible for one location in Sioux Falls, SD.


Documents contact with residents, providers, and families
Maintain individual files on residents
Completes reports according to governing regulations
Pursues avenues for additional services through private, local, state, and federal sources
Creates service management plan
Complies with policies and procedures
Adheres to Fair Housing laws
Utilizes computer, phone, email, and internet as an office tool to assist with communication and information
Tenant Relations and Retention by using rapport to resolve issues and respond to resident needs
Provides limited case management
Promotes wellness activities
Acts as liaison between community agencies, service providers, and residents
