Mercy LIFE Montgomery County a member of Trinity Health PACE, is seeking a full-time Scheduler for its location in Norristown, Delaware.
This customer centered position will work onsite. Scheduled hours will be day shift, Monday through Friday 8 am until 430 pm.
The primary purpose of this position is to coordinate and schedule external appointments for all participants. The scheduler will prepare schedules according to the participant needs and the participant’s care plan designed by the interdisciplinary team and will work closely with the transportation department, participants, caregivers, and center staff.
Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute and long-term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today’s fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers.
Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

Essential Functions include but are not limited to:
· Maintains complete and accurate participant schedules, including the amount, frequency, and duration of service that has been identified in the plan of care. Notifies appropriate manager when requests for scheduling changes are received that require a physician’s order.
· Record all scheduling changes, updates, and deletions on the schedule daily. Respond promptly to participant referrals, oversee service delivery, and monitor participant satisfaction.
· Effectively and accurately coordinate and communicate with all departments within the PACE Organization that are involved with the participant appointments.
· Work closely with clinical leaders to ensure accuracy of all information and to ensure accuracy of all information and to ensure that participant education has been completed by nursing prior to appointments.
· Use the electronic scheduling system (if applicable); schedule Aides services as determined by the Interdisciplinary Team (IDT) and as assigned by the Nursing leader.
· Track and assure all agency personnel have been appropriately trained; obtain all medical clearances from the agency as required by Federal Regulations, prior to caring for PACE Organization participants.
· In the absence of the manager, review agency invoices for accuracy of billed agency hours and forwards onto the Nursing leader for approval, if applicable.
· Create TruChart authorization, transportation appointment, escort appointment, etc.
· Call participant/caregivers and remind them of the next day appointment.
Requirements:

High School Diploma or equivalent required.
Minimum of one (1) year scheduling experience.
Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
Associate degree preferred
One year of customer service experience serving as the first point of contact via email/phone/in person strongly preferred.
Must be action-oriented, have business acumen, manage conflict well, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility. Ability to work with minimal supervision and exercise independent judgment.
Demonstrates superior written and verbal communication and presentation skills appropriate for audience comprehension. Well-developed communication skills, both written and oral, that may be used either in an on-site or virtual environment is required. Able to communicate effectively with individuals and groups representing diverse perspectives.
Excellent organizational skills. Ability to perform multiple duties and functions related to daily operations and maintain excellent customer service skills.
Comprehensive to expert proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system. Knowledge of scheduling systems necessary, experience preferred. Ability to type with speed and accuracy. Ability to use other software as required to perform the essential functions of the job.
Ability to perform frequent detailed tasks and provide immediate service with frequent interruptions. Ability to change and be flexible with work priorities.
Strong problem-solving skills.
Ability to research, analyze and assimilate information from various on-site or virtual sources based on technical and experience based knowledge. Must exhibit critical thinking skills and possess the ability to prioritize workload.
Position may require 10% of travel within the Centers geographic region.
